Make a referral to Kent CAT

To make a referral to the KM CAT service, you will need to complete our referral form (below). Please ensure parents and carers, educational and health professionals involved with the child or young person, have read and understood the Partnership Agreement, before you submit the referral. There is also a Glossary of Terms to help you as well.

Please make sure that all sections of the form are completed, including the parental and educational provision consent. Any forms we receive without full consent can result in a delay of the referral being considered.
